Drop and Withdrawal Form

Please complete the form below to request to withdraw/drop from your online course. Requests received within the drop/add window will be dropped from the student's schedule. All other requests will be processed as withdrawals and must meet the requirements of our withdrawal policy.

All paperwork needs to be signed, include medical doctor documentation (see policy above), and delivered to be considered.  Withdrawals are not considered during the final three weeks of the term.
